*NOTE: By submitting this nomination / application the NOMINEE agrees that if she is selected for WDHOF Membership, that she agrees that within 30 days of receiving notification that she will provide: 1) her official letter of acceptance and notice of whether or not she will attend the Award Ceremony at Beneath the Sea 2008, 2) a brief 100 word biography and 5x7 headshot; and 3) current contact information for the WDHOF Directory.

To submit: 1) In your internet browser window select "FILE"... "SAVE AS" and select "TEXT FILE" (.txt) for the type of file. 2) Select a specific location on your computer and click "SAVE". 3) Open and complete this form (in a text editor) 4) Copy and paste it into an email message, the "Subject" line should read "Nomination 2005 - Your Nominees Name". 5) Send the other required items (biography, resumes, Cvs, letters of recommendation) as "Attachments". These documents must contain file extensions and be compatible with a Windows platform! They can be sent as TEXT (.txt), WORD (.doc) or Acrobat (.pdf) files. If you receive these items in hardcopy format, then you must convert them. We recommend they be scanned at 150 dpi (screen resolution) in black and white and saved as JPG (.jpg) or TIFF (.tif) or Acrobat (.pdf) files.
